The CaRe project intends to cover and identify possible solutions in the “case&care” management: the creation of regional contact points as counselling centres for the system and case management of elderly people in need of care would allow partners to define transnational recommendations to public institutions for the establishment of the management system in the respective regions, as well as define some key topics needed at educational level to train and teach future nurse generations. The project encompass 3 main activities: • Modelling and piloting (WP3): an analysis of the status quo on the welfare systuation of the elderly people in the 3 regions, and the acquisition of relevant data will allow the development of a best practice model, that will be then developed and implemented in the 3 regions with a pilot action of 12 months, in order to understand strengths, weaknesses and possible implementations. • Evaluation and sustainability (WP4): based on the evaluation of the results of the pilot action a report will be elaborated, in order to define some key guidelines for possible application and transferring of the model in other regions, and for the definition of key aspects to be included in university curricula. • Dissemination (Wp2): an awareness raising campaign in each region will support project partners in the dissemination of the results and in informing target groups about the project and its activities.
